LifeLine (mental health case management)

This was a small personal project inspired by my time working as a mental health case manager at a safety net hospital. The various (and numerous) computer systems we used didn't communicate well with one another, and none of them efficiently facilitated monitoring of our most vulnerable clients outside of appointments and the occasional phone call. Lifeline consists of (1) the case manager's dashboard, which allows him/her/them to keep an eye on the most at-risk clients and (2) an app on the client's phone that prompts them to report their mood and medication adherence in just a few seconds. Information entered by clients is relayed instantaneously to the case manager's dashboard. In this way, the case manager is able monitor clients without intrusive phone calls (which generally go unanswered anyway).
