The client, Carilion Clinic underwent a large strategic population health management effort to improve readmissions. As part of that effort, I led a project to streamline and improve the education and engagement of COPD patients system-wide. Using aspects of design thinking and process improvement we interviewed 60+ patients, clinicians and stakeholders to better understand their unmet needs and create actionable insights. The outcome was that over 40 solutions were implemented including EMR education, COPD education for the public, a marketing campaign, additional staff for the rehab center and support groups, more equipment for the rehabilitation center as examples.
